This will be available initially through four primary care networks (PCNs) incorporating over 20 GP surgeries across Poole, Bournemouth Central and Purbeck.

Referrals can be made via GP surgeries by parents/carers or the young person themselves. The sessions can take place in a school environment, at a GP surgery or online.

The initial assessment session explores the challenges and mental health symptoms the young person is experiencing. The young person then works through a guided mental health pathway with the practitioner on changing behaviours, managing symptoms and increasing confidence.

Active Monitoring provides pathways to help support mild to moderate mental health issues such as: Grief and Loss, Anger, Low Mood, Self-esteem, and Anxiety and Stress.

Active monitoring gives young people valuable input into their own wellbeing plan, in a safe and confidential space.

If your GP surgery is not listed that means we are not currently covering that area however please do keep checking as we are hoping to continue to expand; or take a look at our other services offered for children & young people.
